It has been available in its present form since 1976.<\/p>\n<p>Psoralens are compounds found in many plants which make the skin temporarily sensitive to UVA. The ancient Egyptians were the first to use psoralens for the treatment of skin diseases thousands of years ago and nowadays medicine psoralens include methoxsalen (8-methoxypsoralen), 5-methoxypsoralen and trisoralen.[\/vc_column_text][\/vc_column][\/vc_row][vc_row top_margin=&#8221;page_margin_top&#8221;][vc_column][vc_empty_space height=&#8221;125px&#8221;]<h1 class=\"box-header\">Treatment of Different Skin Conditions<\/h1>[vc_empty_space height=&#8221;40px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/3&#8243;]<h2 class=\"box-header\">Psoriasis<\/h2>[vc_empty_space height=&#8221;25px&#8221;][vc_column_text css=&#8221;&#8221;]Psoriasis is a common inherited skin disorder, which may vary considerably in extent and severity. Neither phototherapy nor any other available treatment effects a permanent cure.<\/p>\n<p>PUVA is usually reserved for patients in older age groups, or for those whose psoriasis is either severe or not responding adequately to more conventional forms of treatment. For example, psoriasis with very thick and scaly plaques on trunk and limbs. In approximately 90% PUVA is effective in clearing psoriasis, and can often control it as long as treatments are continued (although this is rarely recommended). Psoriasis in body areas shielded from light (e.g. scalp and body flexures) may not clear satisfactorily with PUVA.[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/3&#8243;][vc_empty_space height=&#8221;55px&#8221;][vc_column_text css=&#8221;&#8221;]Initially most patients have their treatment two or three times a week. The first few exposures will be short (less than 5 minutes). The length of exposure is gradually increased, according to the patient&#8217;s response, up to a maximum of 30 minutes per session. Few patients require such long exposures, most being controlled with shorter times.<\/p>\n<p>Most psoriasis patients will have their psoriasis cleared or much improved after 12 to 24 treatments. At this stage treatments may be reduced to once a week or less. Even without treatment, the skin may remain clear for some months. PUVA can bring about some re-pigmentation, particularly for Vitiligo of the face, and in dark-skinned patients.<\/p>\n<p>Results for other body sites and white-skinned patients are less encouraging.\u00a0Treatment is usually twice a week for two years. When the skin is clear, treatment is given less frequently. If PUVA is stopped, the condition sometimes relapses.<\/p>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/2&#8243;]<h2 class=\"box-header\">Polymorphic Light Eruption<\/h2>[vc_column_text]Polymorphic light eruption (PMLE) is a common light sensitivity disorder. For bathwater PUVA, the patient soaks in a bath containing a solution of psoralens. In most cases, treatment is undertaken two or three times each week.<\/p>\n<p>During treatment, the patient usually stands in a cabinet containing 24 or more 6-foot long UVA fluorescent bulbs.<\/p>\n<p>The patient should always wear goggles to protect the eyes from exposure to the radiation.<\/p>\n<p>Usually, he or she is dressed only in underpants for whole-body exposure. Sometimes just arms and legs may be exposed to the radiation, in which case the patient should wear the same clothing each treatment to prevent inadvertently exposing a new area of skin.<\/p>\n<p>The UVA lamps stay on for increasing lengths of time, starting with about one minute and extending for up to half an hour. There may be fans and\/or air conditioning to cool the cabinet, as it tends to get very warm with more prolonged treatment times.[\/vc_column_text][vc_separator][vc_separator][vc_empty_space]<h2 class=\"box-header\">What is PUVB?<\/h2>[vc_column_text]<\/p>\n<p style=\"text-align: justify;\">PUVB is the combination of Psoralens and UVB (short wavelength ultraviolet radiation). This is rarely used, as the wavelength that activates the psoralens most effectively is in the UVA range.<\/p>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/2&#8243; css=&#8221;.vc_custom_1470155819785{padding: 10px !important;}&#8221;][vc_empty_space height=&#8221;70px&#8221;]<h2 class=\"box-header\">Localised PUVA<\/h2>[vc_column_text]Those patients requiring treatment to small areas only may be treated using a smaller hand and foot unit, and bathwater PUVA may be suitable. In this case, the hands and\/or feet are soaked in a dilute solution of methoxsalen for 30 minutes, then immediately exposed to UVA.<\/p>\n<p>A few patients may be treated with topical PUVA \u2013 a psoralens lotion or gel is applied to the affected areas 10 minutes before UVA exposure.[\/vc_column_text][vc_separator][vc_empty_space]<h2 class=\"box-header\">Can Sunlight Be Used for PUVA?<\/h2>[vc_column_text]Where PUVA devices are unavailable, some dermatologists have recommended patients to be exposed to sunlight after taking psoralens by mouth or applying psoralens topically. The oral psoralens has usually been trisoralen as this is slightly safer than methoxsalen.<\/p>\n<p>Unfortunately, sunlight is unpredictable \u2013 it is difficult to get the correct dose. Too little, and it is ineffective. Too much, and a nasty blistering burn may occur.[\/vc_column_text][\/vc_column][\/vc_row][vc_row top_margin=&#8221;page_margin_top&#8221;][vc_column][vc_empty_space height=&#8221;80px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/2&#8243;]<h2 class=\"box-header no-border\">What is PUVA used For?<\/h2>[vc_column_text el_position=&#8221;last&#8221;]<\/p>\n<p style=\"text-align: justify;\">PUVA may be used to treat various skin disorders, including:<\/p>\n<ul style=\"text-align: justify;\">\n<li>Psoriasis<\/li>\n<li>Dermatitis<\/li>\n<li>Vitiligo<\/li>\n<li>Polymorphic light eruption<\/li>\n<li>Cutaneous T-cell lymphoma<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/2&#8243;][vc_empty_space height=&#8221;70px&#8221;]<div class=\"announcement clearfix vertical-align-table\">\n\t\t\t\t\t<ul class=\"vertical-align\">\n\t\t\t\t\t\t<li class=\"vertical-align-cell\"><h2>Note:<\/h2><p>The number and the frequency of PUVA treatments will depend on the condition being treated and individual factors.\u00a0In general, UVB is also less complicated and less risky.\u00a0Methoxsalen capsules have been difficult to obtain since 2008.<\/p>\n<\/li><\/ul>\n\t\t\t<\/div>[\/vc_column][\/vc_row][vc_row top_margin=&#8221;page_margin_top&#8221;][vc_column][vc_empty_space height=&#8221;55px&#8221;][vc_separator el_position=&#8221;first last&#8221;][vc_empty_space height=&#8221;80px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column][vc_column_text css=&#8221;&#8221;]<\/p>\n<h2>INSTRUCTIONS<\/h2>\n<p>[\/vc_column_text][vc_column_text]<\/p>\n<p style=\"text-align: justify;\">If you are receiving PUVA therapy, follow the advice of our dermatologist. Some general advice:<\/p>\n<p>[\/vc_column_text][vc_empty_space height=&#8221;15px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/3&#8243;][vc_column_text css=&#8221;&#8221;]<\/p>\n<ul>\n<li>Keep all scheduled appointments. Allow time for changing.<\/li>\n<li>Tell the staff about your health problems, including medications and eye disease.<\/li>\n<li>Do not apply any ointments or cosmetics (especially perfumes and coal tar products) except as directed by our dermatologist.<\/li>\n<li>Schedule treatments at the same time of day whenever possible.<\/li>\n<li>Always take the capsules at exactly the same interval before the UVA exposure.<\/li>\n<li>Ideally, take the capsules two hours after a meal.<\/li>\n<li>Alcohol consumption should be minimal.<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/3&#8243;][vc_column_text]<\/p>\n<ul>\n<li>Avoid sun exposure on treatment and non-treatment days.<\/li>\n<li>Apply broad spectrum sunblock to face and hands before treatment.<\/li>\n<li>Cover male genitalia \u2013 this area must not be exposed to UVA.<\/li>\n<li>You must protect your skin and eyes from natural sunlight for twelve hours after taking methoxsalen tablets.<\/li>\n<li>During treatment, you must wear the special goggles provided.<\/li>\n<li>Wear wrap-around ultraviolet-protective sunglasses both indoors and outdoors, from the moment you take the methoxsalen tablets until nightfall on the treatment day.<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/3&#8243;][vc_column_text]<\/p>\n<ul>\n<li>After dark the glasses must still be worn under fluorescent lighting, but are not necessary outside or with incandescent lamps.<\/li>\n<li>Wear fully covering clothing outdoors and if near a window indoors, i.e. hats, long sleeves and skirts or trousers.<\/li>\n<li>Apply sunscreen to all uncovered skin after treatment.<\/li>\n<li>In some circumstances polarized or prescription glasses may be suitable; they must cut out all UV radiation however.<\/li>\n<li>You can use clear Perspex glasses indoors.<\/li>\n<li>Arrange to be seen regularly by our dermatologist.<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][\/vc_row][vc_row top_margin=&#8221;page_margin_top_section&#8221; el_position=&#8221;last&#8221;][vc_column][vc_empty_space height=&#8221;80px&#8221;][vc_separator el_position=&#8221;first last&#8221;][vc_empty_space height=&#8221;80px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column][vc_column_text css=&#8221;&#8221;]<\/p>\n<h2>SIDE EFFECTS OF PUVA<\/h2>\n<p>[\/vc_column_text][vc_column_text]<\/p>\n<p style=\"text-align: justify;\">PUVA has some risks and side effects.<\/p>\n<p>[\/vc_column_text][vc_empty_space height=&#8221;15px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column][vc_toggle title=&#8221;Burning&#8221; el_id=&#8221;1470158251191-7abe5938-7cfe&#8221;]<\/p>\n<ul>\n<li>An overdose of PUVA results in a sunburn-like reaction called phototoxic erythema. It is more likely in fair skinned patients who sunburn easily. A burn is most likely 48 to 72 hours after the first two or three treatments.<\/li>\n<li>Sensitive areas such as breasts and buttocks may need to be covered for all or part of the treatment.<\/li>\n<li>Avoid photosensitizers such as certain oral medications, perfumed cosmetics and applications of coal tar.<\/li>\n<li>If the treated skin becomes pink the dose of UVA should not be increased. One or more treatments may be missed.<\/li>\n<li>Unfortunately, phototoxic erythema can persist for longer than sunburn from natural sunlight. Moisturizers and painkillers can reduce the discomfort.<\/li>\n<\/ul>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Itching&#8221; el_id=&#8221;1470158423888-0e2553ff-3b95&#8243;]<\/p>\n<p style=\"text-align: justify;\">Temporary mild pricking or itching of the skin is common after treatment.\u00a0The skin is often rather dry. Apply a moisturizer frequently. Antihistamine tablets may help.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Nausea&#8221; el_id=&#8221;1470158962493-d36fe85a-b730&#8243;]<\/p>\n<p style=\"text-align: justify;\">It\u00a0occurs in a quarter of those treated with psoralens. If it occurs, tell our dermatologists. Nausea is less if the methoxsalen capsules are taken with food, or the dose is reduced. Antiemetic tablets can be prescribed.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Tanning&#8221; el_id=&#8221;1470159030955-c47bc935-ce7f&#8221;]<\/p>\n<p style=\"text-align: justify;\">PUVA usually leads to tanning which lasts several months. Although the skin appears brown it may still burn easily on sun exposure. Tanning from UVA is not as protective as tanning from combined wavelengths occurring in natural sunlight.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Eye Damage&#8221; el_id=&#8221;1470159059792-44a037aa-a651&#8243;]<\/p>\n<p style=\"text-align: justify;\">If the eyes are not protected from UV radiation, keratitis may occur. This results in red sore gritty eyes and can be very unpleasant. Damage to the lens in the eye leading to cataracts is another possible risk.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Skin Aging and Skin Cancer&#8221; el_id=&#8221;1470159108646-a68c4252-0384&#8243;]<\/p>\n<p style=\"text-align: justify;\">Extensive PUVA treatment results in premature aging changes and can increase the chance of skin cancer.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Dry Skin and Wrinkles&#8221; el_id=&#8221;1470159151324-fdee70e5-9f4f&#8221;]<\/p>\n<p style=\"text-align: justify;\">Discoloration: broken blood vessels, freckles, lentigines.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Squamous Cell Carcinoma, and Less Often, Basal Cell Carcinoma and Melanoma&#8221; el_id=&#8221;1470159190917-93ce7e4e-1a5c&#8221;]<\/p>\n<p style=\"text-align: justify;\">Fair skinned individuals or those with previous sun or radiation damage are most at risk. This is not a concern for most patients, who receive PUVA therapy for two or three months only.\u00a0Patients on long term maintenance therapy should have their skin checked by our dermatologist at least every 6 months. Bring any new moles, sores which are slow to heal, or growing lumps to the dermatologist\u2019s attention.\u00a0Usually, but not always, skin cancers are readily curable. When significant ageing changes are evident or skin cancers occur, it becomes unwise to continue PUVA.<\/p>\n<p>[\/vc_toggle][vc_toggle title=&#8221;Pregnancy&#8221; el_id=&#8221;1470159280220-7c6ba23c-8b2e&#8221;]<\/p>\n<p style=\"text-align: justify;\">There is no evidence to suggest that PUVA will damage a developing baby.
